Binary session log implemented, plus a few related fixes.

New
- binlog keyword: T-Hist v3 binary session log, independent of history.
Details in qico.conf.sample.

Fixes
- [security] binkp: inbound O_PWD only after a matching password.
- inbound log: IPv4 peers as a.b.c.d on the dual-stack, not ::ffff:a.b.c.d.
- freq: drop *.req when no processor is configured / freqtime is closed.

Special thanks to Mikhail Markovskiy for collaboration on the binlog
format and for the statistics work on the T-Hist side (https://github.com/klug378/T-Hist).

See 'Changes' for the full notes.
